About Your Visit

Your visit to our office will most likely require the use of dilation drops.

These drops often blur your vision for extended periods of time.

For your safety, we strongly encourage you to bring an alternate driver or make other arrangements for your ride home.

Please expect your visit to last one to two hours.

Remember to bring:

  • Glasses and/or contact lenses
  • Health insurance cards
  • Medicare/Medicaid cards
  • Referral form when appropriate
  • Social Security Card
  • Driver’s License
  • A list of all medications you are taking with dosage amounts

All patients that have insurance requiring a referral must have the referral to our office one day prior to they appointment.

If you do not have your referral at the time of your appointment, you will be re-scheduled.

Insurance company policies require that any co-pays must be made at the time of service.

Required forms for your visit may be filled out online or printed by clicking on the “Patient Forms”  tab located under Contact Us on our homepage.
